NSAA Guidelines REQUIRE students in grades 8-12 who participate in any athletic related activity (including cheerleading/dance) to have a physical PRIOR to practicing or participating in that activity. Th NSAA/Parent consent form must be completed prior to the beginning of practice. This form is available in PowerSchool (after August 1) and is included in your student's enrollment forms.
Students are REQUIRED to complete the SWAY concussion baseline every year, you may access all necessary medical forms on the school website http://www.mindenwhippets.org. Click on the Menu and open School Nurse. You can download/print forms there.
Practice/Participation will not be permitted until the school nurse receives the complete physical and NSAA/Parental Consent forms (in PowerSchool) and have completed the SWAY protocol for the coming school year.
For your convenience forms will be picked up by Nurse Julie from the Kearney County Medical Clinic OR you can mail them to the school PO Box 301 attn: Julie Anderson, LPN
